How to Become NIFDE Certified

Now that you have made the decision to apply for NIFDE Certification, download the appropriate NIFDE Application and follow the instructions. Remember, the NIFDE Application and Review Process was designed to be time efficient and user friendly!

There are two NIFDE Applications depending upon your situation:

First Time Certification
If you are applying for NIFDE Certification for the first time for your dental facility:



Facility that is Already Certified
If you are a provider applying for NIFDE Certification and you are part of a dental facility already NIFDE Certified:



IF FOR ANY REASON YOU ARE UNABLE TO DOWNLOAD EITHER OF THE ABOVE APPLICATIONS, CONTACT THE NIFDE REVIEW PANEL DIRECTLY AT (609)-951-0105 OR EMAIL enrollment@nifde.com. SHOULD YOU REQUIRE, APPLICATIONS CAN BE MAILED, EMAILED, OR FAXED TO YOUR ATTENTION.

Upon completion of the NIFDE Application or Additional Provider Forms, return the forms and professional fee to:
